What we collect on this site
- Information you type into the contact or consultation request form: name, email, phone, preferred contact method, and a short message.
- Standard server logs: IP address, browser type, pages visited, and timestamps, kept for security and troubleshooting.
We do not use advertising trackers or sell information. TODO(client): state whether any analytics tool is used, which one, and that it is cookie-consented.
How we use it
To reply to your request, to schedule a first appointment, and to keep the site secure. If you become a patient, your contact details are added to your record in our electronic health record system.
Please keep health details out of the form
The website form is for scheduling. Information about symptoms, diagnoses, or medications should be shared only through the patient portal, which is designed to protect it.
Who we share it with
Service providers that host this site and deliver our email under contracts that require them to protect your information. We do not share it with anyone else unless the law requires it.
How long we keep it
Contact requests are kept for TODO(client) months if you do not become a patient, then deleted.
Your choices
You can ask us to correct or delete a contact request by emailing info@lahealthwellness.com. Patients can review their rights in the Notice of Privacy Practices.
