This is a plain-language summary. The full consent is presented in the patient portal before your first visit, and you'll sign it there.
What telehealth is
Care delivered by secure live video and, when appropriate, secure messaging, instead of in person.
What to expect
- Visits happen through the patient portal's encrypted video, not through ordinary video apps.
- You must be physically located in a state where the doctor is licensed during each visit and tell us where you are.
- You will be asked for an emergency contact and your current location in case of an urgent situation during a visit.
Benefits and limits
Telehealth removes travel and makes regular follow-up easier. Some situations still need in-person care, and technical problems can interrupt a visit. If that happens we will call you at the number on file.
Privacy
The same laws that protect in-person care apply to telehealth. Please join from a private place. Visits are not recorded unless you agree in writing.
Your rights
You can stop a telehealth visit at any time and ask about alternatives. Declining telehealth will not affect your right to future care.
